Intraoperative radiotherapy: a report about the commissioning process and risk analysis

Intraoperative radiotherapy is a modality in which a high dose is delivered in a single fraction to the tumor bed. This work aimed to implement this technique through the acquisition and analysis of physical and dosimetric parameters and a risk analysis, by using the FMEA tool. The analyzed parameters were PDP, output factor, dose at the applicator external surface, beam profile, transmission, and backscatter factor of the shields. The output factor increased with the increase in energy and applicator diameter. The dose at the applicator external surface becomes more significant with an increase in energy. The characterization of the transmission and backscatter factors of the shielding discs demonstrated their adequacy. Regarding the FMEA tool, a maximum NPR value of 180 was obtained, with the highest risk priority processes occurring during the surgical procedure. The intraoperative radiotherapy modality was successfully implemented, and FMEA proved to be a useful tool to assist in the creation of a quality assurance program to prevent failures.
