Avaliação dos Modos de Falha do Planejamento Computadorizado em Implantes Intersticiais com Braquiterapia de Alta Taxa de Dose Usando HFMEA

Esse trabalho avalia os modos de falha da fase de planejamento computadorizado em implantes intersticiais com braquiterapia de alta taxa de dose. Foi utilizada a ferramenta de gerenciamento de risco prospectiva Análise de Modo de Falha e Efeito na Saúde (Health Care Failure Mode and Effects Analysis – HFMEA). Foram identificados 12 subprocessos dessa fase e 33 modos de falha, dos quais 21 justificavam novas ações de segurança, sendo 9 riscos intoleráveis. O método mostrou-se útil na identificação dos modos de falha, mas trabalhoso e subjetivo em sua avaliação. Os principais riscos apresentados foram devidos a fatores humanos, o que requer treinamento dos profissionais e empenho da gestão para mitigá-los.
